Should there be a cover over the black (negative) connection like there is with the red?

Generally only the positive terminal has covers in case a wrench is dropped and makes contact with any grounded structure or the negative terminal.

There is no cover on either terminal. You are looking at the bottom of the positive terminal. The terminals point in opposite directions to prevent the chance of arcing.
