Manual surge brake position - Dutchmen TaB

I have a 2007 T@B Wave with mechanical surge brakes in the hitch, which position should the manual brake by wheel crank be in while towing, and does anyone know how to tighten the mechanical brakes? How often should you repack the wheel bearings on the trailer Any help is appreciated.
