I have the same problem. 90% of the time I use a BAL short foot pad instead of the wheel. If I need the wheel, I either have a partner help me lift to put the wheel on, or put the foot pad on - jack it up - block it - and then put the wheel on.
I had this problem and reasoned that, with everything loaded in the bed of my truck, it actually lowered the tongue an inch. I switched to a hitch with 1 inch less drop and everything has been perfect since then. If you're unable to do that, I can give you the details of what I had to do before that, but it is a pain in the toukas.
I make it a habit not to use the safety pin to secure the dollie wheel. That way there is no chance I can forget to remove it which could be very ugly. After I hook to the TV I run the jack up up all the way and the wheel drops off. Yes I did forget to remove the wheel one time which caused some minor damage to the jack. Just something to consider.

Same for me. I think that many people are making the same mistake that I did. We need to measure the ball height when the TV is fully loaded for camping. I increased the height of the ball by one inch and all my problems disappeared.

If I don't need the wheel, I simply put a block of wood under the jack tube to serve as a foot plate.
If I do need the wheel, and can't get it on due to uneven ground, I do as trainwrecked does; jack it up, block it, then install the wheel.
